On the fifth mission, Lt. T.J. McDonald-Breakey made an attack on M.T. together with his No.2 pilot. When in his second strafing dive, Lt. Breakey's machine was observed to turn on to its back at 1,000 ft. and fly straight into the ground.
The aircraft exploded on impact and burned out immediately.No parachute was seen.
